Understanding CFSS

CFSS (Community First Services & Supports) is similar to PCA. CFSS was implemented on October 1, 2024, to allow for changes that will give people more choice and control of how they use services. PCA Services will change to CFSS at your next Assessment.


What Changed?

  • Consultation Services: providers will assist with writing service plan & making informed decisions 
  • Family Members serving as workers: a person's spouse or parent of a minor will be able to provide CFSS services to that person
  • People using CFSS serving as workers: a person using CFSS will be able to provide CFSS services to other people
  • Purchase of goods and services: a person will be able to purchase goods & services to replace the need for human assistance
  • Flexibility: a person will have increased flexibility for distributing units throughout the year

Compassionate Care, Close to Home

Being taken care of and having your needs met in the comfort of your own home can put you at ease. Over 37,000 Minnesota eligible residents are utilizing CFSS/PCA services for the benefit of continuing to be members of their own communities. At Nova Care, we assist Direct Support Workers get paid for taking care of a loved one or someone in need of assistance.

Services

Community First Services & Supports

Our experienced and highly trained Direct Support Workers (DSW's) provide the highest quality of care to both patients and their families. Our goal is to help individuals and their loved ones live happy and fulfilling lives. 

Home Care Assistance

Home care assistance services help individuals live and work within their own communities. DSWs help people who need support to take care of daily living needs because of disease, a disabling condition, or complications related to aging. Our compassionate DSWs can assist you to a healthy and complete recovery.

Nova Will provide your loved ones with the care that they deserve

Our experienced and highly trained Nurses and Direct Support Workers will assist with the follow services:

  • Activities of Daily Living (ADLs) including, but not limited to dressing, bathing, grooming, eating, positioning, transfers and mobility.
  • Instrumental Activities of Daily Living (IADLs) including accompany to medical appointments, accompany to participate in the community, assist with paying bills, communicate by telephone and other media, complete household tasks integral to the CFSS/PCA services, such as planning and preparing meals and shopping for food, clothing and other essential items.
    • A Direct Support Worker (DSW) may assist a child with IADLs when: immediate attention is needed for hygiene reasons integral to the personal care services or when the assessor has indicated this need on the service plan
  • Redirection and intervention for behavior including observation and monitoring
  •  Health-related functions including but not limited to:
    •  Assistance with self-administered medications 
    •  Interventions for seizure disorders, including monitoring and observation 
    •  Range-of-motion and passive exercise to maintain a person’s strength and muscle functioning 
    •  Tracheostomy suctioning and services to a person who uses ventilator support 
    •  Other activities within the scope of CFSS services that meet the definition of health-related procedures and tasks.
